The Disabled Children Register (DCR) is a list of children and young people in Sunderland who have special educational needs and disabilities (SEND).
The information is used to help plan and develop services in Sunderland for families.
You can join the list by creating a user account and then completing an online form. You will have to provide both the parents and the child or young person’s details.
You can be kept up to date about new services or information by letting us know you are happy for us to contact you.
You can ask to have your child or young person’s details removed at any time. They will be removed once they turn 18.
Max Card: Currently anyone registering their child on the register will receive a Max Card. This is a card that offers free or discounted entry to various leisure attractions/venues.
Renewals: If a family require a Max Card to be renewed please send details of yourself, your child/ren (including DOB) and confirmation of your current address by email to fis@togetherforchildren.org.uk.
Please note: applications should not be made via this site by Foster Carers, Personal Assistants, or care workers. This register is ony for parents to register their own children.
