Covid-19 Update - Please note that due to changes in the way staff are working during the Covid-19 pandemic applications to the Disabled Children's Register may take a little longer to process than normal however, we will endeavour to get these processed as quickly as possible. Thank you for your patience.
The Disabled Children Register (DCR) is a list of children and young people in Sunderland who have special educational needs and disabilities (SEND).
The information is used to help plan and develop services in Sunderland for families.
You can join the list by creating a user account and then completing an online form. You will have to provide both the parents and the child or young person’s details.
You can be kept up to date about new services or information by letting us know you are happy for us to contact you.
You can ask to have your child or young person’s details removed at any time. They will be removed once they turn 18.
Currently anyone registering their child on the register will receive a Max Card. This is a card that offers free or discounted entry to various leisure attractions/venues.
Renewals: If you wish to apply for your Max card to be renewed once it has expired, please use the Contact Us link and tell us your and your child's details. Please also ensure you provide us with details of your current address.
Please note - applications should not be made via this site by Foster Carers, Personal Assistants, or care workers. This register is ony for parents to register their own children.
