How can an Education, Health and Care Plan help me?

City Equals is a group representing the voice of young people with learning disabilities or difficulties (LDD) living in Sunderland. 

A number of young people from City Equals worked with staff from Sunderland City Council and a company called Little Big Butterfly to create an information workshop for children, young people and their families.  The aim of the workshop was to give everyone a better understanding of what the changes to the SEN system would mean to them.

John's Story

Find out how Education, Health and Care plans should be used to identify all of a person’s needs in the one plan bringing all elements together.  This example shows how a personal budget can be used to meet John’s social needs by employing a Personal Assistant.

See how a young person who has a Statement of Special Educational Needs at school could transfer to an Education, Health and Care plan which can help the young person find the right pathway into appropriate further education or work experience so that they can develop in the areas they are interested in.
